Community Living Oakville is part of a centralized intake service for the Region of Halton.  Halton Support Services is the initial contact point for families/individuals who are interested in accessing residential and non-residential supports from the Children and Adult Developmental Services Sector in Halton. 

 

A Service Referral Package can be obtained by calling Halton Support Servicesat 905-849-8000 or toll free at 1-800-600-2013. Upon receipt of a completed Service Referral Package, the individual is placed on the Central Referral List which is managed by Halton Support Services.

 

The Information outlined in the Referral Package is shared with a Committee of representatives from the four agencies within the Region of Halton:

  1. Community Living Oakville

Individuals requiring service must have a diagnosis of a developmental disability and have a funding source to cover their share of the cost for residential services.

 

When a vacancy becomes available within one of these four agencies, families are notified. The Regional Committee representing the four agencies within the Region of Halton reviews all people who fit the criteria for the available space. Placement is determined based on individual needs and availability of space.

 

The organization is an employer of many long term employees who have made careers out of providing support to people who have an intellectual disability within Oakville.  The goal is to make it possible for every person, whatever their ability, to live and participate as a full citizen in their community.
