Passport

Community Living Oakville is part of a centralized intake service for the Region of Halton. Halton Support Services is the initial contact point for families/individuals who are interested in accessing residential and non-residential supports from the Children and Adult Developmental Services Sector in Halton.

A Service Referral Package can be obtained by calling Halton Support Services at 905-849-8000 or toll free at 1-800-600-2013. Upon receipt of a completed Service Referral Package, the individual is placed on the Central Referral List which is managed by Halton Support Services.

The Information outlined in the Referral Package is shared with the Halton Coordinated Intact Access Committee, a committee of representatives from agencies within the Region of Halton:

Individuals requiring service must have a diagnosis of a developmental disability and have a funding source to cover their share of the cost for residential services.

When a vacancy becomes available within one of these agencies, a vacancy is declared and the committee meets to place an individual into the home. The committee reviews all applications that fit the criteria for the available space. Placement is determined based on individual needs and availability of space. When a selection has been made, the successful candidate's family is contacted.

Community Living Oakville is an employer of many long term employees who have made careers out of providing support to people who have a developmental disability.  The goal is to make it possible for every person, whatever their ability, to live and participate as a full citizen in their community.

Supported Independent Living (Adult)

Supported Independent Living is an alternative to group home living. This program provides support to individuals to live independently in the community. It focuses on skill development in areas such as banking, cooking, grocery shopping, etc. Support can begin while the person is still living with their family or they could be living in the community already. The amount of support required can vary with each person. Support requirements could be short or long term.

Residential Services

Community Living Oakville provides residential support to adults and youth with developmental disabilities.

Our homes are located in the Oakville area. In general, people receiving support share a home with up to 4 other people and support is provided by staff on a shift-rotation basis.

Supports are person-centred and designed to meet each person’s needs and goals.

After School Program

The After School Program provides a safe and caring environment for individuals 5 to 21 years of age. There are activities for everyone to enjoy; as the program is coordinated to the interests and abilities of individuals. Activities include games, outings, parks, sports, arts & crafts and life skills. Hours of operation are Monday to Friday, 3:00 p.m. to 8:00 p.m.. This is a purchase of service program.

Adult Day Programs, (Alternatives) Life Skills

The Life Skills Wyecroft program is a structured adult day program providing quality activities and life skills to individuals who require more support. On site activities include life skills, cooking and baking, games, pet therapy, music therapy, computer work, math/reading sheets and a bin system. The individuals we support in this program also enjoy many community outings such as bowling, swimming, libraries and grocery shopping.

The Life Skills Church Street Program is an adult day program that provides a variety of activities on site and in the community. Individuals are supported to make choices on planning their day. This program focus is on arts & crafts, music therapy, baking, outings to the movies, malls, parks, bowling and swimming. This program is geared to adults who need a relaxing environment.

Adult Day Programs (Pre-Vocational)

The Pre Vocational program offers adults a day program with a combination of production, work/volunteer experiences and community outings with support. Individuals build their skills with light contract jobs such as assembly and packaging for companies. Individuals are supported with activities such as math/reading sheets, computers, walks, exercise videos, swimming and gardening.

Speers Place Industries

Speers Place Industries provides employment and training for adults with a developmental disability. Working in conjunction with Best Pack, Community Living Oakville’s independently owned packaging business, SPI provides work and the opportunity to develop work skills in a supportive environment. With a program staff of three, SPI employees are highly motivated individuals, ready to work and to learn the skills that will enable them to work both independently and as part of a team.

From basic work ethics and expectations right up to specific skills in packaging, shipping or administration, SPI employees are encouraged to develop their skills to their highest potential and to pursue their employment goals. With that in mind, we offer our employees flexibility and support, allowing them to take advantage of opportunities as they arise, whether they be community-based employment, educational or volunteer situations while still maintaining the support provided by SPI.

ODSP Employment Supports and Supported Employment

Support is provided to individuals in job searches, job training and maintaining employment in the community. Job Coordinators will work with the individual and the employer to help the person achieve successful employment within the Oakville area. Job Coordinators can also provide the necessary support for a volunteer/work experience, work training and or situational assessment. Application for the program can be provided through the Job Coordinators. Individuals who apply for the program must be unemployed at the time of the application and seeking a job that pays minimum wage or more.

Passages

PASSAGES provides a Resource Coordinator who assists individuals and their families with transition planning. The Resource Coordinator can assist participants with the transition from school to work opportunities, community-based activities and community living supports. The Resource Coordinator also facilitates regular information sessions related to facilitating choices regarding future life directions. Participants in this program have the opportunity to attend the Employment Training Centre during their high school years and to follow curriculum that assists with life planning and skill development.

Partnerships have been established with the Halton Catholic District School Board and the Halton District School Board in order to build on the skills of participants and to provide exposure to a variety of opportunities.

Resource Coordinators can assist with referrals to community programs and services within Halton region.

STEPP Program

The STEPP (Skills Training to Enable Personal Progress) program was established in October 2006. The program was created to meet the needs of individuals with a developmental disability who had recently graduated from high school and did not have access to meaningful daytime activities.

The goal of the STEPP program is to facilitate independence, social skills and communication skills, while fostering the development of literacy and numeracy skills. Community participation and awareness are an important part of our social skill development.

This program has expanded to five days a week from 9-3. This is a fee for service program.

Passport

Individuals may choose to use Passport funding to purchase supports from Community Living Oakville. Community Living Oakville will work with the individual and their support network to create a person-centered program that meets the needs and wants of the individual within the parameters of their Passport funding.
